Recommended dosing and adjustments for each procedure1

1 mL of JELIDO® = ~20 mg lidocaine hydrochloride

Urethral anaesthesia1

Surface anaesthesia of the adult male urethra

Usual dose: 20 mL

  • Instill ~10 mL of JELIDO® until the patient experiences a feeling of tension
  • Apply a penile clamp at the corona for several minutes
  • Then, instill the remaining 10 mL of JELIDO®

When anaesthesia is especially important (e.g., during sounding or cystoscopy), 30 to 40 mL of JELIDO® may be required

  • Instill in 3 to 4 portions and allow to take effect for 10 to 12 minutes before insertion of instrument
  • JELIDO® instilled into the bladder is also effective for procedures in this region

To anaesthetize only the anterior urethra (e.g., for catheterization), small volumes (5 to 10 mL) of JELIDO® are usually adequate

Endoscopy1

Usual dose: 10 to 20 mL
Small amounts of JELIDO® may also be applied to the instrument

Surface anaesthesia of the adult female urethra

Usual dose: 5 to 10 mL

  • If desired, JELIDO® may also be deposited on the orifice and covered with a cotton swab
  • Allow JELIDO® to take effect for several minutes before insertion of instrument

Lubrication for endotracheal intubation1

Apply ~2 mL to the external surface of the endotracheal tube just prior to insertion

  • Care should be taken to avoid introducing JELIDO® into the lumen of the tube

Do not use JELIDO® to lubricate the endotracheal stylettes

Proctoscopy and rectoscopy1

Usual dose: Up to 20 mL

Catheterization instructions1

  • Clean the urethral area before using JELIDO®
  • Remove the syringe from its sterile package
  1. Icon showing a gentle press on syringe plunger while the cap is on

    STEP 1

Prior to removing the syringe cap, gently press on the plunger to release the pressure point.

  2. Icon showing removal of the cap

    STEP 2

    Remove the syringe cap.

  3. Icon showing eye examining a small amount of jelly.

    STEP 3

Force a small amount (i.e., 1 cm) of JELIDO® out of the syringe.

Inspect the syringe and jelly to ensure that there is no particulate matter, precipitation, discolouration, or leakage prior to administration.

Do not administer if you see particles, discolouration, or leakage.

  4. Icon showing application of gel to the male urethraIcon showing application of gel to the female urethra

    STEP 4

    MALE/FEMALE

Apply a small amount of gel around the urethral opening. Then insert the syringe tip into the opening of the urethra and depress the plunger slowly and smoothly to insert the gel.

  5. Icon indicating a 5-10 minute waiting period

    STEP 5

Allow at least 5 minutes for the onset of anaesthetic action. Certain procedures may require longer contact times.†

Maximum dosage1

Adults

  • Dependent on the application site
    • Oral: 20 mL (400 mg)
    • Urethra and bladder: 40 mL (800 mg)
  • Maximum single dosage for JELIDO® has not been established
  • No more than 4 doses should be given during a 24-hour period

Children

  • Children need to be closely observed during and after use of topical anaesthetics as they are at greater risk than adults for serious adverse events (e.g., methemoglobinemia)
  • 2–11 years of age
    • Dose varies based on age and weight
    • Dose should not exceed 6 mg/kg of body weight or 3 mL per 10 kg body weight
  • 12–18 years of age
    • Doses should be commensurate with weight and physical condition
  • No more than 4 doses should be given during a 24-hour period

Additional information1

  • Special populations: Debilitated patients, acutely ill patients, and patients with sepsis should be given reduced doses commensurate with their age, weight, and physical condition because they may be more sensitive to systemic effects
  • When JELIDO® is used concomitantly with other products containing lidocaine, the total dose contributed by all formulations must be kept in mind
    • The total dose of lidocaine should not exceed 400 mg
  • Syringes are for single use only. Any gel not used in a single application should be discarded
  • The syringe tip does not support needle attachment

JELIDO® important safety information

Indications and clinical use:

JELIDO® (lidocaine hydrochloride jelly) is indicated for:

  • Surface anaesthesia and lubrication for:
    • The male and female urethra during cystoscopy, catheterization, exploration by sound, and other endourethral operations
    • Nasal and pharyngeal cavities in endoscopic procedures such as gastroscopy and bronchoscopy
    • Proctoscopy and rectoscopy
    • Tracheal intubation
  • Symptomatic treatment of pain in connection with cystitis and urethritis

Pediatrics (2–18 years of age): Children should be given reduced doses commensurate with their age, weight, and physical condition.

Pediatrics (< 2 years of age): Lidocaine should be used with caution in children younger than two years of age as there are insufficient data to support the safety and efficacy of this product in this patient population at this time.

Geriatrics (> 65 years of age): Elderly patients should be given reduced doses commensurate with their age and physical condition.

Contraindications:

  • Patients who are hypersensitive to this drug or to any ingredient in the formulation, including any non-medicinal ingredient, or any component of the container
  • Patients with a known history of hypersensitivity to local anaesthetics of the amide type or to other components in the formulation
  • Patients with congenital or idiopathic methemoglobinemia
  • Infants ≤ 12 months of age who require treatment with methemoglobin-inducing agents (e.g., sulfonamides)
  • Patients with glucose-6-phosphate dehydrogenase deficiency, which makes them more susceptible to drug-induced methemoglobinemia

Most serious warnings and precautions:

  • PATIENTS SHOULD BE INSTRUCTED TO STRICTLY ADHERE TO THE RECOMMENDED DOSAGE: EXCESSIVE DOSAGE, OR SHORT INTERVALS BETWEEN DOSES, CAN RESULT IN HIGH PLASMA LEVELS OF LIDOCAINE OR ITS METABOLITES AND SERIOUS, AS WELL AS LIFE-THREATENING, ADVERSE EFFECTS, including methemoglobinemia.
  • Use in children and infants (< 3 months): Ensure the caregiver understands to limit the dose and area of application and to prevent accidental ingestion. Children should be closely observed during and after use of lidocaine as they are at greater risk than adults for serious adverse events (e.g., methemoglobinemia).

Other relevant warnings and precautions:

  • Lowest effective dose should be used
  • Avoid contact with eyes
  • Potential triggering agent for familial malignant hyperthermia
  • In patients under general anaesthesia who are paralyzed, higher plasma concentrations may occur than in spontaneously breathing patients
    • Unparalyzed patients are more likely to swallow a large proportion of the dose, which then undergoes considerable first-pass hepatic metabolism following absorption from the gut
  • Endotracheal tube lubrication
    • Avoid introduction of the jelly into the lumen of the tube
    • Do not use to lubricate the endotracheal stylettes
  • Topical anaesthetics used in the mouth may enhance the danger of aspiration
  • Numbness of the tongue or buccal region may enhance unintentional biting trauma
  • May temporarily impair locomotion or alertness
  • Use with caution in patients with:
    • Sepsis
    • Traumatized mucosa at the area of application
    • Epilepsy
    • Severe shock
    • Bradycardia
    • Impaired cardiovascular function
    • Hepatic disease or impaired renal function
    • Known drug sensitivities
  • Exercise caution when driving and operating machinery
  • Exercise caution with use in all porphyric patients
  • Reduced doses should be given in debilitated patients, acutely ill patients, and patients with sepsis
  • Care should be taken during early pregnancy
